From CFSCG to Centers Health Care

Historical CFSCG pages describe a consortium of affiliated care organizations across New York City, Long Island and upstate New York. Their service descriptions included nursing, rehabilitation, physical therapy, cardiac care and other inpatient and outpatient services. The website distinguished the group from the individual organizations providing care at its listed locations.

New York’s Department of Health was already identifying Centers Health Care as formerly Centers for Specialty Care Group in material for the Public Health and Health Planning Council meeting of 26 March 2015. The same record described administrative services and clinical and administrative consulting for care facilities. It therefore documents the connection between the historical and current names.

The network and its care providers

Centers Health Care’s current website describes a network of skilled nursing, rehabilitation and senior-care services. Its contact address is in the Bronx. The site also explains that Centers Health Care supplies administrative and business support to affiliated providers, while affiliated facilities and companies are independently owned and operated.

That distinction matters when identifying a particular nursing home, rehabilitation center or service company. An affiliation with the Centers network does not by itself identify the legal owner or licensed operator of a facility. The current disclosure expressly separates Centers Health Care from ownership or operation of healthcare providers and managed care plans.

Development of the care continuum

The organization’s own timeline traces its beginnings to Williamsbridge Manor in the Bronx in 1996, later called Williamsbridge Center. It dates the addition of home health care to 1999, adult day care to 2009, a dialysis unit at Bronx Center to 2010 and an assisted-living program at Argyle Center to 2014.

The present contact directory includes skilled nursing, home health care, adult day care, renal dialysis, laboratory services, telemedicine, urgent care and business support. These are distinct service settings within the wider network. The appropriate facility or service-company listing provides the starting point for checking what is offered at a particular location.

Locations and current information

The current center list includes Boro Park, Bronx, Brooklyn, Bushwick, Fulton and Richmond centers, among other New York locations. Historical names need to be read in their dated context: a 1 September 2017 announcement records the change from Waterfront Center in Buffalo to Ellicott Center for Rehabilitation and Healthcare.

The current Centers Health Care website describes its geographic footprint as New York and New Jersey and provides a center finder and service contacts. Older CFSCG location lists remain useful for understanding the network’s history; the current directory is the relevant source for finding today’s facility names and contact information.
